Record of Medication Order:
This document is optional. Any over the counter medication like acetaminophen, Ibuprofen, diaper creams/ointments, Chapstick, sunscreen etc. must be on the medication order and signed by your child’s physician (Any items we have to apply or administer that do not have a prescription). Medication orders do need updated annually.
Physical and Immunization:
The physical must be signed by a physician, physician’s assistant or nurse practitioner and is only needed upon enrollment. Immunizations should be kept current throughout enrollment in our care, as the child ages.
